1. Services Provided

Nurture Sleep PLLC provides sleep medicine consultations, tele-sleep services, diagnostic evaluations, and related health services delivered through secure telehealth platforms in compliance with applicable laws and regulations. In person visits are also done on appointment basis,

• You should be present in your state of official residence during the time of Tele-visit

• Insurance claims may be submitted only if we are credentialed with your plan. You remain financially responsible for uncovered charges.

• Missed or late-canceled appointments may incur fees.
